After power cycling rack, crate 14 connected to the XL3 server fine (the normal message was printed), however it did not connect to Orca (no message about crate 14 connecting). I did a hard reset at the ML403 in case something weird happened and the same issue when the crate reconnected to the xl3-sever. I telnet'd into the xl3-sever and was able to successfully reset the crate.

After restarting Orca the issue appeared resolved.

I doubt we will ever be able to reproduce this, since its the first time I've ever seen this, but creating this issue primarily for documentation purposes.
